Government of National Capital Territory of Delhi
Health & Family Welfare Department
9th Level, A-Wing, Delhi Secretariat, New Delhi – 110002

Number: sshfw/78 Date: 28.04.2020

The COVID-19 pandemic is a threat to the well-being of people in Delhi. Testing and tracking all fresh positive cases as well as the tracking of repeat samples is critical for the state to ensure immediate monitoring of all cases. If this information is not collected from laboratories in a time-bound manner, it will lead to delay in tracing new positive cases. This could be a serious threat to public safety. In view of the need to track the COVID-19 testing reports of all samples being processed by labs in Delhi, the Department hereby directs all labs to provide reports to the Integrated Disease Surveillance Program Delhi via email in the following form:

1. Complete Report of every first test that is positive, along with the scanned Requisition Form

2. Complete Report of every patient undergoing a repeat test, along with the scanned Requisition

Form Labs are to send these documents in addition to the line lists that all labs are currently sending to IDSP.
